ABSTRACT
Congenital coronary anomalies (CCA) are rare but important conditions with a wide spectrum ranging from normal variants with no functional significance to potentially life-threatening anomalies. Both CT angiography and cardiac MRI (CMR) are noninvasive imaging modalities for the visualization of coronary arteries. With increased spatial and temporal resolution and reduced radiation doses thanks to technological advances, coronary CT angiography provides comprehensive, rapid anatomical isotropic three-dimensional (3D) imaging of the entire coronary artery system. Although CMR is advantageous because it does not require the use of contrast media and does not contain radiation, it is often inadequate for coronary artery imaging beyond the ‘whole heart’ proximal artery. Especially in older children with larger coronary arteries, contrast-enhanced CMR can better visualize the ‘whole heart’. With the imaging methods preferred according to the situation, we radiologists should be familiar with variants and clinically important anomalies in order to make the correct diagnosis and prevent unwanted errors during surgical-interventional procedures.
