Çocuklarda Yanık Yara Enfeksiyonu, SIRS ve Sepsis

ÖZET

Çocuklarda yanık yaralanması dünya çapında önemli bir epidemiyolojik sorun olarak gözlenmekte ve tüm yanık yaralanmalarının yaklaşık dörtte birinin, çoğunluğu beş yaşın altında olmak üzere 16 yaşın altındaki çocuklarda meydana geldiği bildirilmektedir. Yanık yaralanmalarında bağışıklık sistemi üzerinde meydana gelen değişiklikler sonucunda vakaların enfeksiyonlara duyarlılığı artmaktadır. Yanık yaralanmaları nedeniyle hastaneye yatışın erken evrelerinde yara ve yumuşak doku enfeksiyonları ön planda gözlenebilirken, daha sonraki evrelerde kan dolaşımı enfeksiyonları hatta sepsis tablosu gelişebilmektedir. Yanık yaralanmasında, bağışıklık sisteminin zayıflaması ve cilt bariyerinin bozulması, başta bakteriyel ve çoklu ilaca dirençli mikroorganizmalar olmak üzere mantar ve viral enfeksiyonlara karşı duyarlılığın artmasına neden olabilmektedir. Sonuç olarak, yanık yaralanmalarında enfeksiyonlara yatkınlık arttığı gibi, farklı mikroorganizmalarla yara enfeksiyonundan sepsise kadar değişebilen geniş spektrumda enfeksiyon tablosu ile karşılaşmak mümkündür.
