ABSTRACT

Coronary venous pathologies are a rare and underrecognized group of conditions that may arise in isolation or as a component of systemic disease. Unlike coronary arteries, veins lack muscular media and are less prone to primary atherosclerotic changes, but they are vulnerable to iatrogenic injury, inflammation, thrombosis, and infiltration by systemic diseases or neoplasms. This chapter reviews the pathological spectrum of coronary venous involvement, including thrombotic lesions, vasculitis, granulomatous inflammation, tumor infiltration, and amyloid deposition. Although often incidental or secondary findings, these pathologies can contribute to myocardial dysfunction, arrhythmias, and graft rejection. Greater awareness and standardized reporting of coronary venous lesions in pathology practice may enhance diagnostic accuracy and improve understanding of their clinical significance.
