ÖZET
Egzersize bağlı anafilaksi (EIAn), fiziksel efor sırasında veya kısa bir süre sonra anafilaksinin meydana geldiği nadir bir klinik sendromdur. İki hastalığı içermektedir: egzersize bağlı anafilaksi ve besin ilişkili egzersize bağlı anafilaksi (FEIAn). Her iki hastalığın da altında yatan patofizyolojisi henüz iyi bilinmemektedir. EIAn ve FEIAn tanısı klinik hikayeye dayanmaktadır. Tanı için egzersiz provokasyon testi gerekli değildir, ancak EIAn’yi taklit eden diğer durumlar dışlanmalıdır. FEIAn’da, suçlu besine karşı IgE duyarlılığı deri prik testi ya da in vitro immünoassay ile gösterilmelidir. Hastalığın yönetimi, erken belirtiler hakkında hastanın eğitimi, egzersiz, suçlu besinler ve tetikleyici kofaktörlerden kaçınmayı içerir. Tüm hastalar her zaman yanlarında adrenalin otoenjektörü taşımalıdır. Olgu sunumlarında, kromolin sodyum, antihistaminikler, misoprostol ve omalizumabın EIAn/FEIAn semptomlarının önlenmesinde faydalı olduğu bildirilmektedir.
