ABSTRACT
Molar incisor hypomineralization (MIH) is characterized by defective enamel, which is most fundamentally related to the tooth’s predisposition to caries and pulpal and periapical tissue diseases. For severely MIH-affected teeth, although extraction and molar substitution are recommended, these teeth may need to remain due to reasons including hypodontia or anchorage. Due to a deficiency of regular dentist check-ups, in these teeth, which are prone to post-eruptive enamel breakdown, pulp pathologies may be observed. Therefore, endodontic treatment requirements are frequently encountered. MIH-affected teeth demonstrate variations in root and canal morphology. Qualitative and quantitative changes such as increased nerve fibers are observed in the pulp. This may make it difficult to apply vitality tests. The mineral component of dentin is changed, and abundant bacteria are observed. Considering the necessity of maintaining these teeth in function, the endodontic treatment should be performed after obtaining detailed root and canal anatomy knowledge, considering factors such as severity, future orthodontic treatment, and parental attitude.
