ÖZET

Total eklem artroplastisi (TEA), dejeneratif eklem hastalıklarının tedavisinde önemli bir yer tutmakta ve artan yaşlı nüfusla birlikte bu prosedüre olan talep de yükselmektedir. Bununla birlikte, TEA’nın ciddi bir komplikasyonu olan PEE, nadir görülmesine rağmen önemli bir sağlık sorunu oluşturmaktadır. PEE, özellikle biyofilm oluşumu nedeniyle tedavisi güç olan enfeksiyonlardır ve hastanede uzun yatış sürelerine, revizyon cerrahilerine ve ekonomik yüklerin artmasına yol açar. PEE insidansı, TKA ve TDA gibi farklı artroplasti tipleri için çeşitli coğrafi bölgelerde ve hasta gruplarında değişiklik göstermektedir. Enfeksiyon oranları, özellikle cerrahi sonrası ilk yıl içinde yüksek olup, revizyon cerrahilerinde de enfeksiyon riskinin daha fazla olduğu gözlemlenmiştir. Enfeksiyon tedavi maliyetleri primer artroplastilere göre belirgin şekilde daha yüksek olup, revizyon gerektiren vakalarda ekonomik yük daha da artmaktadır. Son yıllarda yapılan çalışmalar, enfeksiyon riskini azaltmak için antibiyotik yüklü kemik çimentoları gibi yenilikçi tedavi yöntemlerini araştırmaktadır. Ancak, bu gelişmelere rağmen enfeksiyon oranları endişe verici düzeyde kalmaktadır. PEE’nin tedavi maliyetleri, sağlık sistemleri üze- rinde büyük bir yük oluşturmakta olup, bu durumun, artroplasti sonrası enfeksiyon oranlarını azaltmaya yönelik daha etkili önleyici stratejilerin geliştirilmesi ihtiyacını ortaya koymaktadır.
