ÖZET

Sakroiliak Eklem (SİE), sakrum ve ilium arasında yer alır ve omurgayı pelvik kemiklere bağlar. Anteriorda sinovyal eklem ve posteriorda sindesmoz olmak üzere iki bölümden oluşur. SİE bozuklukları, toplumun yaklaşık %15’inde bel ve kalça ağrısının kaynağıdır. SİE ağrıları artrit, spondiloartropati, travma veya malignite gibi eklem kaynaklı olabileceği gibi ligaman yaralanmaları, miyofasyal ağrılar, gebelik, fraktür ve yansıyan ağrılar gibi eklem dışı nedenlere bağlı olarak da görülebilir. Radyolojik değerlendirmede konvansiyonel radyografi, bilgisayarlı tomografi (BT), kemik taramaları ve manyetik rezonans görüntüleme (MRG) gibi çeşitli görüntüleme teknikleri kullanılır. SİE bozukluklarının tanısında spesifik provokatif testlerden (Fleksiyon-abdüksiyon-eksternal rotasyon-ekstansiyon (FABERE), Kompresyon, Distraksiyon, Uyluk İtme ve Gaenslen testleri) en az üçünün pozitif olması gereklidir. SİE kaynaklı ağrıların tedavisi oldukça kapsamlıdır. Tedavide medikal ilaçlar, fizik tedavi ajanları, eklem içi enjeksiyonlar, radyofrekans ablasyonu, proloterapi gibi cerrahi dışı veya SİE füzyonu gibi cerrahi yöntemler kullanılır.
