ABSTRACT
The somatic nerves are a division of the peripheral nervous system and allow for the voluntary control of body movements via skeletal muscles. The somatic nervous system consists of both afferent neurons and efferent neurons. Mechanical compression of somatic nerve roots is classically accepted as the cause of radiculopathies, leading to pain, weakness, and paresthesia. However, the increased use of MRI to characterize the location and degree of disc disease and nerve compression has demonstrated incongruence between imaging and degree of symptoms. Many patients with marked discopathies may be asymptomatic. Similarly, many patients may undergo surgical decompression without resolution of pain. It is likely that mechanical compression of the nerves by time causes an inflammatory process ending with radicular pain. Nerve blocks with local anesthetic are a common diagnostic tool to evaluate the cause of radicular pain, allowing the physician to directly evaluate a nerves involvement by the degree of pain relief afforded. At this chapter, under US and fluoroscopic guided two different techniques related with somatic nerve block were presented.
