ÖZET

Akut aort sendromları (AAS), yüksek mortalite riski taşıyan ve acil tanı ile müdahale gerektiren klinik tablolar bütünüdür. Terim ilk kez 2001’de tanımlanmış olup üç ana alt grubu içerir: akut aort diseksiyonu (AAD), intramural hematom (IMH) ve penetran aortik ülser (PAU). AAD, AAS vakalarının çoğunu oluşturur (%80-90). Bu sendromların en tipik semptomu göğüs veya sırt bölgesinde hissedilen, genellikle bıçak saplanır tarzda ani başlayan şiddetli ağrıdır. Bu sendromlar ilerleyerek ciddi komplikasyonlara yol açabildikleri gibi, gerileyebilir veya rezorbe olabilirler. Tanı ve tedavi sürecinde acil servis ekibi, kardiyoloji, kalp-damar cerrahisi, yoğun bakım ve radyoloji uzmanları multidisipliner bir yaklaşım sergiler. Erken tanı ve hızlı sevk, mortaliteyi azaltmada kritik öneme sahiptir. Kesin tanıda kontrastlı bilgisayarlı tomografi anjiyografi (BTA) altın standarttır. Ancak transtorasik ve transözefagial ekokardiyografi, manyetik rezonans görüntüleme ve akciğer grafisi gibi yöntemler de tanı koyma aşamasında önemli rol oynar.

Aort hastalıklarının tarihi oldukça eskidir. Aort hastalıkları ilk kez Eski Mısır’da Ebers Papirüsü içinde ‘aort tümörü’ şeklinde tanımlanmış, Hint tıbbında ‘Granthiden’ olarak anevrizmalardan bahsedilmiştir. Hipokrat ve Galen ani ölümleri tarif etse de, aort patolojilerini açık şekilde ayıramamıştır. 16. yüzyılda Andreas Vesalius, aort diseksiyonunu (AD) sistematik biçimde tanımlamıştır. 18. yüzyılda İngiltere Kralı II. George’un ani ölümü sonrası yapılan otopsi ile AD’nin ölümcül etkileri ortaya konmuştur. 19. yüzyılda Virchow, damar duvarının patolojik süreçlerini açıklayarak diseksiyonun temel mekanizmalarına ışık tuttu. 20. yüzyılın ilk yarısında tanılar çoğunlukla otopsiyle konulmaya devam ederken, Carrel’in vasküler sütür çalışmaları ve DeBakey-Cooley’in 1955’teki ilk başarılı cerrahisi dönüm noktaları olmuştur. DeBakey ve Stanford sınıflamaları klinik karar verme sürecinde yaygın kullanıma girmiştir.

1980 sonrası bilgisayarlı tomografi, manyetik rezonans ve ekokardiyografi ile tanı daha hızlı şekilde koyulambilmiş; 1990’larda endovasküler onarım (EVAR) uygulamaları cerrahiye alternatif oluşturmuştur. 2001’de AAS kavramı modern terminolojiye girmiş, uluslararası kayıt sistemleri (IRAD) klinik verileri güçlendirmiştir. Günümüzde AAS yönetiminde multidisipliner ekiplerin, aort merkezlerinin ve uluslararası kılavuzların katkısıyla mortalite önemli ölçüde azalmıştır. Yapay zeka destekli tanı yöntemleri ve hasta temelli bireyselleştirilmiş yaklaşımlar ön plana çıkmaktadır.
