ABSTRACT

Transverse malocclusions are a prevalent challenge in orthodontic practice, observed in both anterior and posterior regions of the dentition. Posterior crossbite, characterized by a buccal-lingual discrepan cy of opposing molars or premolars, is one of the most common type of this malocclusion. Early di agnosis and treatment are essential to prevent long-term complications such as mandibular asymmetry and developmental deficiencies in the maxilla. Clinical evaluations, dental cast analyses, and two or three-dimensional imaging are critical for accurate diagnosis and treatment planning. Clear aligners, an increasingly popular alternative to fixed orthodontic appliances, are particularly effective in achiev ing dental arch expansion for mild to moderate malocclusions, providing significant advantages such as improved esthetics, patient comfort, and oral hygiene maintenance. These appliances achieve dental arch expansion primarily through buccolingual tipping, effectively reducing crowding and achiev ing aesthetic and functional outcomes. However, the success and predictability of treatment planning depend on patient compliance, precise treatment planning, and biomechanical considerations. This chapter provides a comprehensive overview of transverse malocclusions, including their etiology, diagnosis, and treatment strategies. It highlights the role of clear aligners in managing transverse mal occlusions, supported by evidence-based insights into their predictability and limitations. Significant biomechanical principles, such as overcorrection, root torque control, and proper attachment place ment, are discussed to guide clinicians in optimizing outcomes. Dependency on patient compliance, potential posterior open bites and sensitivity to oral conditions are some limitations of clear aligners. Despite these disadvantages, clear aligners offer significant advantages such as improved aesthetics, better oral hygiene, reduced treatment duration and chair time. Success of clear aligners in treatment of transverse malocclusions is remarkable, particularly in expanding the dental arch, improving peri odontal health, and achieving a stable, functional occlusion.
